	Volume of Lines
	Type of Organization (FQHCs, RHCs and hospitals located in MUAs are entitled to a special discounted rate)
	Contract Term (Minimum of two years required for discounted rates)
	Prompt Payment (i.e. Payment within 30 days from Date of invoice required for special discount eligibility).
